Community First Services and Supports (CFSS)

Community First Services and Supports (CFSS) is a self-directed service that provides individuals with more control over their care. This program is designed to help people with disabilities and special health care needs live independently in their homes and communities.

 

Key Features of CFSS:

  • Flexible Support: Services can be provided at home or in the community.
  • Comprehensive Assistance: Includes help with daily living activities, health-related tasks, and instrumental activities of daily living (IADLs).

Assistance with ADLs and IADLs
CFSS provides support with Activities of Daily Living (ADLs) such as:
  • Bathing
  • Dressing
  • Grooming
  • Eating
  • Mobility
  • Transferring
  • Toileting
Additionally, CFSS covers Instrumental Activities of Daily Living (IADLs) like:
  • Meal preparation
  • Medication management
  • Housekeeping
  • Laundry
  • Shopping for groceries and essentials
  • Transportation to medical appointments
  • Managing finances
Who is Eligible?
CFSS is available to individuals enrolled in Minnesota Health Care Programs (MHCP) who need assistance with daily living activities due to disabilities or special health care needs.
