The Minnesota Personal Care Assistance Program

The Minnesota Personal Care Assistance program provides services to individuals who need help with day-to-day activities to allow them be more independent in their home and community.

If you qualify for Medicaid in MN and require services that are medically necessary, you should contact your public health nurse to arrange an assessment that determines what services you qualify for. A case manager will then be assigned to help you find the perfect company and care plan to address your needs.
